New Accounts
Account requests need to be sent in by an authorized individual in order to be created. If it is not known who needs to authorize the request, users can:
- Have their manager or supervisor reach out to Strata staff (strata-support@vitalhub.com) and an agent can forward the request to the appropriate individual
- For Nova Scotia, users can consult the Nursing Homes and Residential Care Facilities Directory or if an NSHealth employee, reach out to ProvincialePlacementforLTC@nshealth.ca
- For AHS Physio/HealthFirst, if it is unknown who is the authorizer for the location in question or if the authorizer needs to be updated - contact the local PT Community email:
- CommunityPT-Calg@albertahealthservices.ca
CommunityPT-Central@albertahealthservices.ca
CommunityPT-Edm@albertahealthservices.ca
CommunityPT-North@albertahealthservices.ca
There are several exceptions where account management is completed internally by the local health authority or requests may not need LRA approval.
Exceptions include:
- Island Health: All accounts with @islandhealth.ca domains should request Pathways access internally through the Clinical Solutions Desk
- Email: IMIT.GCS.ClinAppSupport@islandhealth.ca
- Phone: 1 (250) 370-8777
- Fraser Health Authority (FHA): all Pathways requests or questions can be sent to pathways@fraserhealth.ca
- Resource Matching and Referral (RMR) - for all CLHIN locations only account requests should be directed to jdelacruz@oakvalleyhealth.ca. TCLHIN locations can send in requests to their internal IT or have their supervisor send in to Strata.
- Search PAC accounts - any user can request access for staff directly to support desk (strata-ussupport@vitalhub.com)
- US Case Manager or Hospital Admin accounts - Includes Search, Honor Health, Sarah Bush Lincoln - Ask CSM (Cody Shaw, Greg Weber, etc.) to authorize unless user is known to request accounts
- Honor: Ryan Dill, Jamie Humphreys
- SBL: Denise Foley
